WALK THE TALK – November 10-19, 2010
WAIVER & RELEASE STATEMENT: By registering, you agree, warrant and covenant as follows: 1. I AGREE to comply with the rules and regulations, and the event instructions of WALK THE TALK. 2. I UNDERSTAND that participating in one or more days of the 10-day-round-the-island walk maybe a potentially hazardous activity and can result in serious injury or death. I am aware of and I expressly assume all inherent risks associated with participating in this event, including the effects of weather, high heat and humidity, traffic, and the conditions of the road. 3. IN CONSIDERATION of your accepting this entry, I for myself and anyone entitled to act on my behalf, waive and release from any and all claims for injuries and damages I may have against the Walk the Talk, H-5, the City & County of Honolulu, State of Hawaii, the United States of America, the sponsors, their agents and representatives caused by the negligence of them arising out of my participation in this event, including pre and post walk activities. 4. I ATTEST that I am physically fit and have sufficiently trained for completion of some or all of the ten-day legs. 5. I CONSENT to receive medical treatment which may be advisable in the event of illness or injuries suffered by me during this event, and I agree to pay for the costs of my medical treatment. 6. I GIVE PERMISSION for the free use of my name, voice or picture in any broadcast, telecast, advertising promotion or other account of this event. This event cannot accommodate minors 10 years old and under. Minors 11-17 years old must be accompanied by an adult.
